As of the end of the day on the 31^st May 2025, the Review closed to new cases. This means that we are no longer accepting new families into the Review. The Trust will identify cases that meet the Review’s Terms of Reference up until 31^st May and will notify the Review of these cases as soon as they can; this means that these families will receive initial contact from the Review team after the closing date of the 31^st May. The Review team will continue to: review family cases and write personalised family feedback, meet with families, and staff, analyse their findings, write the final report. Gillian Lyons

Gillian has been a registered midwife for 27 years and is currently working at The Royal Wolverhampton NHS Trust. After 4 years as a qualified rotational midwife Gillian was appointed as Risk Manager/ Specialist Midwife and has been in this role for the last 23 years.

She works relentlessly to promote high standards of care and has many years of experience in leading investigations following a serious incident. Gillian is key in implementing patient safety incident response framework (PSIRF) within maternity and is currently actively involved in thematic reviews to identify trends and shared learning. Gillian works closely with the maternity neonatal safety investigation team (MNSI) and actively shares the learning with all staff.
